Mandatory Requirements:

Original or Certified Birth Certificate

Participant

Registration fee- Cash, check, Visa or Master Card accepted
(Tackle - $160.00, Cheer - $115.00 and Flag - $85.00)

Copy of final report card - ( May registrants will turn in copy at first practice)

Proof of Residence - Gas, Electric, Water bill - ONLY!!!!!

Physical Form:
It Must be completed and turned into Scottsdale Pop Warner NO LATER than the first practice, or the child will be unable to participate until the physical is received. Only those on an official Pop Warner form and stamped by the doctors office will be accepted.
